Enrollment Form
You will need to know...
The Enrollment Form is to be completed by a:
- LEOFF eligible individual
- WSPRS eligible individual
- PERS elected official (person appointed by the Governor; city manager; or chief
administrative officer of a city, town, port district employee, or public utility district)
The member returns the form to the employer. The employer completes section two and returns
the form to DRS.
