An important reminder about beneficiary forms

You gather lots of information when you hire a new employee. And while it may seem like some of that information may not be needed immediately, the beneficiary form you ask employees to fill out for DRS is. If your employees return the form to you, please ask them to send it directly to DRS instead.

Your employees may also need to update their beneficiary information from time to time throughout their careers. Life changes that could change an employee's beneficiary include marriage, divorce or the death of a loved one named as beneficiary.

We must have that information on file to provide retirement benefits to members' beneficiaries if an unforeseen event occurs. Your employees' beneficiary designation is not valid until it is fully complete, signed and received by DRS. The form instructs employees to submit it directly to DRS, not to their employers. The DRS beneficiary form is available online.
