We are in the process of printing a limited supply of the Plan Choice Booklet. This new supply will have a 9/2008 version date on the back. We expect to have a new supply by the end of September.
Both the 6/2008 and 9/2008 versions include current member contribution rates and references to the replacement of Horizon Funds. We need you to ensure new employees eligible for Plan Choice have access to this information.
We recommend you provide a link to the current Plan Choice Booklet to anyone still in their 90 day decision window if they were given a pre-06/2008 version. Please recycle any version with a date prior to 6/2008 – this will ensure new employees have the most current information for their plan decision.
If you need to order Plan Choice Booklets, we request that you order only the amount that you expect to use through the end of this year. We are currently in the process of revising the Plan Choice Booklet and will send you a new supply in December for new employees as of 2009.
