Member Information Form

You will need to know...

The Member Information Form is used by:

  • Plan 2 members who choose to transfer to Plan 3
  • Plan 3 members returning to eligible employment
  • New members of SERS hired from July 1, 2007 through July 31, 2009 with no prior PERS Plan 2 membership
  • New members of TRS and PERS
  • New members have 90 calendar days to choose Plan 2 or Plan 3 or default.

The member returns the completed form to the employer.

  • The employer completes section 5, enters the information into the employer payroll system, and mails the original to DRS if section 2 was required.
